Office Policies

1. CANCELLATIONS-NO SHOWS-RESCHEDULING.

Please give at least 24 hours notice for appointment cancellations or rescheduling. This helps us offer your spot to someone on the wait list. You will be charged for appointments not cancelled or rescheduled in advance, but exceptions may be made for unavoidable circumstances.

We do not reschedule late cancellations or no-shows for initial evaluations. Patients who cancel less than 24 hours before an appointment or miss their visit will be charged a $75 fee.

2. APPOINTMENTS THAT BEGIN LATE.

If you arrive late, your appointment will still end at the scheduled time to keep other patients on track. If the providers are behind schedule, your session will last its normal duration.

If you arrive after your session has ended, we may accommodate you if there’s a cancellation or no-show, but this is not guaranteed. Otherwise, it will be considered a no-show.

3. EMERGENCY APPOINTMENTS.

We will strive to accommodate crisis situations and early appointment requests. If no appointments are available right away, please check with us daily for cancellations. Similarly, we cannot ensure immediate rescheduling of missed appointments.

In case of a crisis, it is advisable to visit Psychiatric Urgent Care or the nearest Emergency department.

4. FEES FOR SERVICES PROVIDED OUTSIDE OF SCHEDULED APPOINTMENTS

Please inform us at the start of the session if we need to discuss or complete forms, notes, or letters. Fill out forms prior to the session to maximize our time. Activities requiring attention outside scheduled appointments will incur a fee of $50 per half-hour, excluding clinically essential tasks such as interacting with other clinicians involved in your care, providing refills, routine authorization requests, and brief patient phone calls.

5. PRESCRIPTION REFILLS

We prefer to refill medications during office visits. Provide at least 48 hours’ notice for refill requests. Chronic medication refills require an office visit. For other refills, contact your pharmacy; they will fax a request to our office.

6. MEDICAL RECORDS

To transfer your medical records to a different health care provider, you must sign a Release of Medical Records form. The recipient’s name, address, and phone number must be included on the form.
